Current State of Being of our members. We focus on hygiene, Activities of Daily Living
(ADLs), existing life plans, support needs to manage chronic conditions, and various
other aspects specific to senior care. These crucial details are then thoroughly reviewed.
In-Depth Review and Recommendations: After completing our comprehensive
assessment, our team conducts an in-depth review of the findings. We identify areas
where support is needed and provide a rationale for early engagement to prevent
potential decline. Based on these insights, we offer a variety of personalized options
for members to explore and implement, along with the appropriate resources.

Golden Years Planning Assessment - Recommendations - Plan: 5-6 hours, and 2 hours dedicated to working with the member. One time Service.
In addition to the above services, we help create a Golden Years plan. We provide
impartial evaluations of at-home situations to help families plan appropriately for their
loved ones' care needs. Our personalized plans ensure a fulfilling and well-coordinated
long-term care journey for you or your loved ones. Our comprehensive planning benefits
members, families, and care teams by ensuring efficient and coordinated long-term care.

Healthy Ship Program: In addition to either of the above services offered, you may wish to continuing working with your personal Steward in our Healthy Ship. This program offers ongoing support through
monthly connections, ensuring continuous and personalized care for our members. (1 hour +) Our services include:
• Addressing Care Recommendations and Barriers: We work with existing
or enhanced care teams to implement care recommendations and
overcome any barriers.
• Navigating Insurance and Community Resources: We assist members in
understanding their insurance coverage and accessing valuable
community resources.

Care Coordination: We help address care needs and advocate on behalf of our members, ensuring they receive coordinated, effective, and compassionate care.

Support for Members' Care Teams: Our detailed assessments identify unknown needs, stabilize member conditions, and help with future aging plans if declines are noted.

Monthly Outreach / Support: $200 / month after the initial month.

2. Healthy Ship Service
A. Minimum Due: This includes the selected Assessment/Golden Year services and the first month of the Healthy Ship service.

smooth start to your care plan.
